- ONLINE [EASIEST & FASTEST!]: Use the form below to upload all current vaccination documentation (rabies & bordatella/kennel cough), updated license (if not lifetime), acknowledge your acceptance of our liability waiver, and make your secure membership payment via credit/debit card or PayPal.
- EMAIL: Email proof of current rabies & bordatella/kennel cough vaccination as well as updated license (if not lifetime) to [email protected]. Please make sure the subject of your email includes your full first and last name. Make your payment online below OR mail a check to CADPA at PO Box 1016, Carlisle, PA 17013, OR leave a check in the white lockbox at the park.
