Incident Report Form
We all hope that incidents and accidents will never occur…but it is essential that any incidents that occur at the park (personal injury, dog aggression, theft, property damage, etc) be reported for our records AND so the Board can take appropriate action. It’s our goal to solve problems as quickly and painlessly as possible, to the benefit of all concerned, as well as to address any general park issues to improve safety and overall experience.
The CADPA Board MUST have a completed Incident Report in order to pursue appropriate action. If you aren’t sure whether your experience merits a report, feel free to reach out to a Board member or email us at [email protected].
